- Biometric photographs (White Background – 5x5cm) (Visa form should have the same photo)
- Travel details for the last 5 years.
- Request letter by the applicant.
- Passport (valid for at least 6 months beyond your planned arrival in Turkey).
- Travel document along with copies of every page of the recent and old passports (If lost, an attested police report and its English translation may be required).
- Travel health insurance certificate with a minimum coverage of €30,000 or $50,000.
- Polio vaccination certificate from an authorized government medical facility.
- Copy of flight reservation/booking.
- Copy of hotel reservation (reservation only) or invitation letter indicating accommodation details if applicable.
- Family registration certificate from the NADRA office (if family members are traveling).
- If the applicant is a business owner, a statement on company letterhead about the purpose of the visit.
- If the applicant is an employee, a statement from the employer on company letterhead indicating title, salary, duration of employment, and purpose of the visit, along with the last 3 months’ salary slips.
- FBR/NTN/Income/Active Tax Payer Certificate(s) for individuals and companies obtained recently from the relevant Tax Offices.
- Bank account maintenance certificates for personal and company accounts, along with original bank statements for the last three (06) months (with sufficient funds).
